Review Health Assessments

Review Health Assessments (RHA) – are undertaken 6 monthly for children aged under 5 years, or 12 monthly for children and young people aged 5 until your 18th birthday.

These are carried out by your allocated Lead Health Professional, a Health Visitor, School Nurse or a Children in Care Nurse.

Your Lead Health Professional will contact your carer to arrange this appointment which will be carried out either where you are living, at your school or a venue of your choice.

The Health Assessment will look at all parts of your wellbeing. This includes your physical health, your feelings, and your social wellbeing. It is a chance for you and your carer to talk about any worries or concerns and get support if you need it.

It is important that you or your carer brings to the appointment any important information you may have about your health. This will include information about specialist services and appointments you may be attending such as dentist, opticians, hearing, speech and language, Child and Adolescent Mental Health Services (CAMHS) or any other relevant health services.

After the assessment, your nurse will write up the agreed care plan. This plan will explain how to support your health and wellbeing, and who will help you. The plan will be shared with your consent with your carer, GP, and social worker. You can also ask for a copy to be sent to you.
