The Children In Care teams offer support to all care leavers up to age 25, irrespective of whether you are engaged in education or training or not. You can contact our service up to the age of 25 and request support.

What happens when I leave care?

When you are leaving care (aged 16-18 years) you will be provided with a health summary of your relevant personal health information to support you moving into adulthood and make positive decisions about your future health.

Information will be given to you, with an opportunity to discuss it with the Looked after Children’s Nurse. You will be able to continue to access health advice and services and know how to do so.

My last health assessment

The Children In Care Nurses will see you at your last Health Assessment and will continue to be available for support.

The Local Offer for Care Leavers can be found on the Essex County Council which you can access by clicking here

This offer tells you about the support available in Essex for you as a care leaver. This offer has been put together with the help of local young people leaving care and other agencies.

It explains how you can contact your Children in Care team (CIC team) and where you can get help, further information and support, including tenancy and housing issues and includes health as well as education, training and employment.

At 16-18 years you will have an allocated Leaving & After Care Social Worker and/or Personal Advisor, and so your social work team will change to the Leaving & After Care team.
